There are a number of reasons why the references are not linked to the content when a new version of a document is uploaded and the user brought all annotations forward:

New Version is a scan or screenshot
The user is able to bring forward annotations, but this does not link to the content as the system does not recognize scanned text. The user should manually link the annotation to the content.

The text is changed (Examples)
Previous Version with linked annotation New Version without linked annotation
The references are brought forward, however they are not linked since the actual word or selected text is changed. This is also the case when the text is translated into a different language (for example, for localization). The user should manually link the annotation to the content.

The same word or text (where reference is linked to) is twice on the same page
Previous Version with linked annotation New version without linked annotation
The reference is brought forward. However, it is not linked, as the system detected two of the same texts on the same page. The user should manually link the annotation to the content.
