Question:

Why are tables and/or figures not appearing in the bookmarks for a document in Vault?

Answer:

The Sequence (SEQ) Fields for these tables and figures do not have the expected space after the opening '{' bracket, and before the closing '}' bracket.  These SEQ Fields need to have these spaces or Vault is not be able to identify the tables and figures properly.
