Version History of a document

Lilly believes correction needs to occur w/n version history of a document.  Currently, the reviewer/approver listed for each version of a document shows all reviewers and approvers since the document was created, it's keeping a running list of whose in that group w/n the system.  Every system I've had experience with to date captures who reviewed/approved a specific version of the document. 

If an auditor asks who approved a specific version of a document, it should be as easy as looking at the version history of a document.  The individual shouldn't have to run the workflow history report, decipher which start date is associated with a specific version to answer the question.  Lilly believes this functionality, although is operating as expected from design perspective is wrong and needs to be corrected.
