We understand below from Veeva support team - 

Approach – 1: Choose Account à Create a Call of Event Type à Add Attendees later à Survey will be visible (Offline and Online)

Approach – 2: Create Event à Create a call from the Event à Add attendees later à Survey will not be visible (Offline) but Survey will be visible (Online)

Can you please consider Approach - 2 also for making this available offline as it is available online.
