Answer:
If the User is authenticating through Azure, the error below could be occurring due to the Authorization Server Client ID containing an incorrect value within the OAuth profile in Vault.
Verify the Authorization Server Client ID value from the OAuth app registration page in Azure and enter it into Vault.
